The Utah Trucking Association Driver of the Month Program is a prestigious initiative designed to recognize and celebrate the exemplary contributions of Trucking Association member drivers. This program seeks to highlight individuals who not only maintain a spotless driving record but also exhibit a positive attitude and engage in outstanding deeds within their communities. By acknowledging the exceptional efforts of these drivers, the program aims to promote a culture of excellence and community involvement within the trucking industry. To nominate a deserving driver, individuals are encouraged to fill out the application below, providing details about the nominee's driving record, attitude, and noteworthy community involvement. Additionally, nominators are invited to upload a photo to help showcase the outstanding individuals who make a positive impact both on the road and in their communities. The Utah Trucking Association Driver of the Month Program stands as a testament to the commitment of its members to safety, professionalism, and community service.
Basis for Nomination: It is the Intent of the Utah Trucking Association and the Utah Safety management Council to select a driver who will, by past and future record, exemplify the type of career whose high standards of conduct and ability are both an asset to themselves and a credit to the trucking industry in the state of Utah. Outstanding deeds and involvement in the community will be given due consideration. The major terms to be considered in selecting the driver to be honored will be: 1. Past driving record. 2. Loyalty and exceptional attitude toward employer and fellow emplyees. 3. Personal appearance. 4. Respect for and care of employers property and equipment. 5. Letters of commendation from the general public or from law enforcement agencies. 6. Good citizenship. 7. Community service. (What does the driver do when not driving? Volunteer, coach, etc.)
Please submit any additional letters of recommendation or testimonials about the driver. This is optional, you are not required to submit any additional documents. We are no longer accepting copies of the drivers MVR, please do not attach it to this application.
